В статье представлено клиническое наблюдение аденовирусного поражения почечного аллотрансплантата с ультразвуковой картиной динамических изменений при комплексном мониторинге почки, включая интраоперационное ультразвуковое исследование. Пациенту 6 лет с диагнозом “фокальный сегментарный гломерулосклероз, терминальная стадия хронической почечной недостаточности” выполнена операция аллотрансплантации почки от родственного донора 54 лет. Функция пересаженной почки немедленная. В послеоперационном периоде на 19-е сутки при комплексном ультразвуковом исследовании диагностирована дисфункция трансплантата, предположительно в результате интерстициального криза отторжения. В этот же день при пункционной биопсии трансплантата были выявлены изменения, которые характерны для поражения почки аденовирусной инфекцией. На 25-е сутки данные ультразвукового исследования свидетельствовали о тромбозе вен периферического сосудистого русла, что было подтверждено при морфологическом исследовании удаленного почечного трансплантата. Выполнена повторная трансплантация почки от умершего донора. Посттрансплантационный период протекал благополучно.

Case report of adenovirus infection in renal transplant patient is presented. 6 years old patient with focal segmental glomerulosclerosis and stage 5 chronic kidney diseases underwent renal transplantation from the 54 years old living related donor. Kidney function started immediately. Transplant dysfunction was diagnosed on 19th day after the surgery during ultrasound investigation. Adenovirus infection was revealed during renal transplant biopsy.
