Обследовали 201 срединный нерв у 122 пациентов в возрасте от 26 до 70 лет с клинико-электрофизиологическими признаками синдрома запястного канала и 110 локтевых нервов у 88 пациентов в возрасте от 20 до 74 лет с клинико-электрофизиологическими признаками кубитального туннельного синдрома. В качестве контроля обследовали 135 срединных нервов у 84 здоровых пациентов в возрасте от 25 до 70 лет и 141 локтевой нерв у 91 здорового пациента в возрасте от 24 до 77 лет. Площадь поперечного сечения нерва измеряли с помощью ультразвукового исследования в предварительно регламентированных сегментах: S1 - дистальный отдел нерва, S2 - выход из туннеля, S3 - туннель, S4 - вход в туннель, S5 - проксимальный отдел нерва. Специалист, проводящий ультразвуковое исследование, не был осведомлен о результатах электронейромиографии. Значения AUC сегментов в области туннеля (синдром запястного канала - от 0,787 до 0,930; кубитальный туннельный синдром - от 0,776 до 0,869) значительно превысили аналогичный показатель удаленных сегментов (синдром запястного канала - до 0,591; кубитальный туннельный синдром - до 0,666). После объединения наиболее чувствительных сегментов в один сегмент S2-4max получили более высокие значения AUC, используя в качестве показателя максимальное значение площади поперечного сечения нерва, найденное на объединенном отрезке. Методика ультразвуковой диагностики карпального и кубитального туннельных синдромов может быть оптимизирована за счет поиска максимального значения площади поперечного сечения на отрезке, объединяющем все сегменты нерва в области туннеля (S2-4max), что позволяет получить более высокий уровень диагностической эффективности, чем при оценке каждого сегмента в отдельности. Выявленные пороговые значения максимальной площади поперечного сечения нервов в области туннеля можно использовать для диагностики синдрома запястного канала (норма - ≤12 мм2, патология - &gt;12 мм2 (чувствительность - 91,0%, специфичность - 97,8%, точность - 93,8%, AUC - 0,974)) и кубитального туннельного синдрома (норма - ≤12 мм2, патология - &gt;12 мм2 (чувствительность - 83,6%, специфичность - 95,0%, точность - 90,0%, AUC - 0,928)).</p></abstract><trans-abstract xml:lang="en"><p>Aim of the study was to optimize ultrasound nerves examination technology in upper limb tunnel neuropathies. 201 median nerves (122patients aged 26- 70 years old with clinical and electrophysiological signs of carpal tunnel syndrome) and 110 ulnar nerves (88patients aged 20-74 years old with clinical and electrophysiological signs of cubital tunnel syndrome) were investigated. The control group included 135 median nerves (84 healthy volunteers aged 25-70 years old) and 141 ulnar nerves (91 healthy volunteers aged 24-77 years old). Cross-section area was measured at distal part of nerve (S1), at level of tunnels exit (S2), at level of tunnel (S3), at level of entrance to tunnel (S4), and at proximal part of nerve (S5). AUC values for segments within tunnel in carpal tunnel syndrome ranged from 0.787 to 0.930; in cubital tunnel syndrome - from 0.776 to 0.869. They were significantly higher than in remote segments (carpal tunnel syndrome - up to 0.591; cubital tunnel syndrome - up to 0.666). After merging segments, which had the highest sensitivity, in the single segment S2-4 max, and using maximum value of nerve cross sectional area in this merged segment as a cut-off, higher AUC value was achieved.
